BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2013009551 DESCRIPTION A 51-year-old female patient presented to the emergency room with a 3-month history of increasing dyspnoea, and 5 kg weight loss. Her medical history was not relevant. Physical examination revealed 100/90 mm Hg blood pressure, 98 bpm, orthostatic hypotension, a soft grade 2/6 systolic murmur at the left sternal border and bilaterally pulmonary rates. ECG showed normal sinus rhythm with left atrial enlargement. A transthoracic echocardiogram (figure 1A,C,D) revealed a large atrial myxoma occupying the majority of the left atrium, which prolapsed through the mitral valve during the entire cardiac cycle (videos 1, 3, 4 and 6). Grade II mitral regurgitation, severe mitral stenosis gradient and severe pulmonary hypertension were noted (videos 2 and 5). Left atrial myxoma excision (figure 1B) and pericardial patch implantation was performed with good recovery and no signs of heart failure to date.

Effects of two new dihydropyridine derivatives, mepudipine and dibudipine, on isolated human internal mammary artery and rat left atrium

Among the present classes of calcium channel blockers, dihydropyridine derivatives are widely used in the therapy of hypertension, angina pectoris and the other cardiovascular diseases. Since the prototype of dihydropyridine derivatives, nifedipine, does not have the optimum pharmacokinetic and pharmacodynamic characteristics, several attempts have been made to synthesize other drugs in this ...

Use of contrast echocardiography in diagnosis of anomalous connection of right superior vena cava to left atrium.

A 4-month-old infant with cyanosis but without other abnormal cardiac findings is presented in whom the diagnosis of anomalous systemic venous connection to the left atrium was made by contrast echocardiography. The diagnosis was later confirmed by cardiac catheterisation and selective cineangiography. When saline was injected into a vein on the dorsum of each hand while echocardiographically r...

Stab wounds penetrating the left atrium.

A stab wound penetrating the left atrium (excluding the left auricle) would have to penetrate through other chambers of the heart or through the great arteries at their origin from the heart to reach the left atrium from the front of the chest. Such wounds would be fatal, the patient dying from profuse haemorrhage within a few minutes. A stab wound through the back of the chest could reach the ...

Giant left atrium.

Radiographic evidence of asymmetrical enlargement of the left atrium without atrial infarction is presented. Giant atrial enlargement is rarely symmetrical, the atrial appendage can contribute to the enlargement, and the giant atrium can be effectively trimmed. This will reduce its size and, therefore, the space which it occupies, and will restore the anatomical relations of neighbouring struct...

Non-smoke spontaneous contrast in left atrium intensified by respiratory manoeuvres: a new transoesophageal echocardiographic observation.

OBJECTIVE To elucidate why different types of contrast appear in the left atrium during transoesophageal echocardiographic contrast studies. This should lead to a more uniform definition of true patent foramen ovale. BACKGROUND The Valsalva manoeuvre and cough are routinely used to enhance right to left shunt for the detection of patent foramen ovale. No information is, however, available on ...
